Before your initial appointment you will receive an email with instructions on how to access this secure web site. Completing your medical history survey on-line via the Patient Portal will allow us to provide you with more prompt and comprehensive care.
No more hand written questionnaires, histories, or surveys that must be completed year after year. You can now enter your history once, in the convenience and privacy of your home and take as much time as you need.
Although this tool is designed to be a convenient resource for our patients, it should not take the place of any important communications about your care. We continue to encourage you to call our office as necessary.

What is the Greenway Patient Portal
Greenway Patient Portal is a free, password-protected and web-based interactive service that allows you to securely access and track your health information. Using your personal computer or smart phone, you can sign in to Prime Patient Portal and see your test results, medication you are taking and a summary of your current health conditions.

How do I sign up?
During registration you will be asked for your personal email address. Within a few days, you will be sent an email with a link to the Greenway Patient Portal web page. Once you click on the link and follow the prompts to set-up your account, you will receive and email with your username. You will then be able to sign into your account.

Is my information safe?
Yes. You are the only one who can access your Portal. A timeout feature protects your information if you leave the portal page open. Username and passwords are required.

What should I do if I forget my password?
To reset your password, click here or go to the log-in screen and click on “Forgot Password?” You will be asked your username and questions that identify you. A new link to re-set your password will be emailed to you.

When can I see my test results in Greenway Patient Portal?
In most instances, your test results are released to your Greenway Patient Portal account after your physician has reviewed them. This is generally within two days to two weeks. Some routine test results are automatically delivered to your account, while others are released by your health care provider. In both cases, your provider will receive and review all your test results, and he or she will contact you if further discussion about your result is needed.
